Requirements:

Application letter addressed to the University President through the CEO Engr. Audy R. Quebral, DPA 2. Duly accomplished PDS (CS form 212, revised 2017) with work experience sheet if applicable 3. Photocopy of Performance Rating in the last two (2) rating periods covering one (1) year performance in the current/latest position, if applicable; 4. Photocopy of valid and updated PRC License/ID if applicable 5. Photocopy of Certificate of Eligibility/Rating if applicable 6. Photocopy of Transcript of Records (OTR) and Diploma, including completion of graduate and post-graduate units/degrees if appliable 7. Photocopy of Certificates of Completion/Attendance/Participation to Relevant Training/seminars/conferences if applicable 8. Photocopy of Certificate of Employment, if applicable 9 Other relevant credentials. All documents received on or before Dec 29, 2023 are the only documents to be assessed during the comparative assessment.
